How Dental Coverage Tiers Work

Most PPO dental plans sort treatments into three buckets. Preventive care like exams and cleanings is usually covered in full, basic work like fillings and simple extractions is often covered around 80% after the deductible, and major work like crowns and bridges is commonly covered around 50%. The plan stops paying once you hit the annual maximum, which then resets the next plan year.

Why the Annual Maximum Matters

The annual maximum is the single biggest limit on a large treatment. If a crown and an implant land in the same year, the cap can mean insurance covers the first and little of the second. Sequencing care across two plan years, when the dental situation allows it, is a common and legitimate way to use more of your benefit.
